and participating in Mothers Across America Inc.
individual and/or group training, activities and events is a potentially unsafe activity. I am voluntarily
joining and accept all risks associated with participating in
team activities including, but not limited to, injuries, sickness
and death, and harm to my child if present at training sessions.
I agree not to participate if I have any medical limitations which may
increase risk of injury, sickness or death. In the event of medical
emergency I grant access to all medical records and physicians.
I give permission to Mothers Across America Inc. and Charlotte
Gould to use any photographs, motion pictures or any other record
of my participation in this event or related activities for any
legitimate purpose without remuneration. I, for myself or anyone
entitled to act on my behalf, waive and release Mothers Across
America Inc. and Charlotte Gould from present and future claims
and liabilities of any kind, known or unknown, arising out of
my participation in this team and related activities and events. |
